Regular Full Time

POSITION SUMMARY

Tsawwassen First Nation (TFN) is seeking a dedicated Community Wellness Worker to join the Family Services team. This position provides culturally grounded, relationship-based support to Indigenous individuals, youth, and families through prevention, intervention, and post-vention services that promote wellness, healing, and connection to community.

Working within a trauma-informed, strengths-based, and harm-reduction framework, the Community Wellness Worker supports individuals and families through outreach, advocacy, wellness planning, system navigation, and coordinated care. The role assists community members in accessing services and support before, during, and after treatment, while promoting safety, stability, and overall wellbeing.

As part of a multidisciplinary Family Services team, the Community Wellness Worker helps strengthen protective factors, build life skills, support recovery and reunification, and foster meaningful connections to culture, family, and community. Through relationship-building and holistic support, the position empowers TFN members to achieve their goals and enhance their long-term wellness.

DUTIES AND RESPONSIBILITIES

  • Provide pre/post-care coordination and case management for members returning from treatment, corrections, hospital care, or other support settings
  • Develop and implement individualized wellness, recovery, and reintegration plans that reflect member goals, strengths, and needs
  • Deliver culturally safe, trauma-informed, family-centered, and harm-reduction-based services that support long-term wellness and healing

  • Provide education and awareness regarding substance use, mental health, harm reduction, healthy coping strategies, and family support
  • Identify and respond to emerging safety concerns, crises, relapse indicators, and barriers to success while coordinating appropriate interventions and supports
  • Conduct outreach and build trusting relationships with members through respectful, culturally grounded, and non-judgmental engagement
  • Collaborate with internal departments, community agencies, healthcare providers, treatment centers, and other partners to ensure coordinated service delivery
  • Participate in case consultations, interdisciplinary planning, community committees, and working groups related to health, wellness, and substance use
  • Monitor program outcomes, maintain statistical data, and contribute to program evaluation, development, and continuous quality improvement
  • Maintain accurate, timely, and confidential documentation, case notes, reports, and client records in accordance with TFN policies and applicable privacy legislation
  • Remain informed of current social issues, substance use trends, treatment approaches, and best practices through ongoing professional development
  • Contribute positively as a member of the Family Services team and perform other related duties as required to support the health, safety, and wellbeing of TFN members and families
