Wage: AFOA-aligned salary range commensurate with experience
The Elders and Wellness Support Coordinator supports Skeetchestn Elders through home management, housekeeping, home visits, social services and non-medical support services. The position also delivers cultural, social and recreational programming and/or arranges travel to such activities for Elders who do not have access to transportation. Supports coordination patient travel.
Key Responsibilities
Performs all duties and responsibilities in accordance with Skeetchestn Indian Band policies, practices, and procedures as directed by the Manager, Community Service. Maintains confidentiality as required pertaining to members, clients, employees, and general matters of SIB.
Elders Program
- Visits elders, in friendship, to provide general support and companionship.
- Acknowledges birthdays and special dates.
- Organizes Elders birthday luncheons, field trips and plan the annual elders gathering.
- May assist elders with transportation and support for day to day errands (i.e. haircuts, shopping)
- Provides support during community emergencies.
Elders Home Support
- Perform light housekeeping based on client needs.
- Ensures home is a safe environment.
- Visits clients and provides caring, friendly support. Listens to clients needs and assists clients to navigate and access appropriate resources.
- Monitors general state of health and informs appropriate health team professionals of any client health concerns.
Patient Travel
- Assess patient travel eligibility based on FNHA guidelines and assists members with completing necessary paperwork.
- Provide community members with travel assistance to and from the Skeetchestn clinic, and occasionally schedule group transportation to and from Kamloops, working closely with the clinic to coordinate.
- Prepares quarterly and annual reports to FNHA.
- Follows Skeetchestn procedures for purchases orders, patient travel expense claims and credit card use procedures.
- Maintains all records and files for the patient travel program.
Other related duties as required.
Qualifications
Training, Education, Experience
- Grade 12.
- Experience working with Elders.
- Homecare experience.
- Current First Aid Level 1 Certification
- WHMIS Certification
- Current Class 5 BC Driver’s Licence.
- Satisfactory criminal records check (vulnerable sector).
Knowledge, Skills, Abilities
- Enjoys working with Elders and community members and contributing to their health and wellness.
- Strong active listening skills and the ability to build positive caring relationships built on trust.
- Knowledge of the Shuswap language is an asset.
- Highly-developed communication and interpersonal skills.
- Strong cultural sensitivity and understanding. Able to demonstrate respect and caring without judgement.
- Knowledge of the history of Indigenous people in Canada and the impact of historical trauma.
- Ability to work independently and as part of a collaborative team.
- Ability to exercise a high degree of professionalism and confidentiality.
- Ability to establish and maintain positive relationships with clients, community members and co-workers.
- Office administration skills including general record keeping.
- Basic to intermediate skills computer skills including MS Office.
- Genuine respect for Skeetchestn Indian Band culture and protocols.
Working Conditions
Work is performed in a variety of setting including office, Elder’s homes and other community settings. Physical fitness to perform light housekeeping (standing, stooping, reaching, lift lifting). Driving within the community and some travel outside the community for patient travel, Elder support, attending meetings or training is required.
