Case Manager - FAMILY STEPPING STONES

Family Stepping Stones (FSS) is a program within Child Home Services; one of the many services offered by New Directions. Family Stepping Stones offers supports for families who struggle with parenting due to various factors (e.g., substance use, mental health, lack of resources, trauma, lack of supports, etc.) and whose children are in care of CFS or at risk of coming into care. Family Stepping Stones creates a team with the parents, CFS, relatives, significant others, and the children to create a healthy nest for the children and develop plans with the family. The values that guide life in the home, the routines, and plans are set with the parents and/or relatives based on their culture and traditions. The home is also the parents’ home during the times when the parents are able to engage in routines, teachings, guidance, and support for their children. No sustained period is required, it is based on the functioning in the moment and the goal is to help build parenting skills and support meaningful attachment.

 

 

Responsibilities include: (but are not limited to) 
  • Ongoing training and development of support workers and Home Supervisors
  • Attend monthly clinical consults
  • Case management
  • Main point person for all youth/child correspondence with social workers and outside resources/school, planning etc
  • Resource referrals  
  • Maintain and update case notes, reports and other administrative tasks as assigned 
  • Review incident reports and address concerns
  • Participate in monthly team meetings 
  • Participate in the On-Call rotation

 

Qualifications:

  • Applicants must have post-secondary education in a related field.   B.S.W. /B.A. is considered an asset
  • Preference will be given to individuals with related social services education with a minimum of 4 years’ experience
  • Related experience may include some or all of the following working with individuals who may have Intellectual and/or developmental disabilities, mental health issues, individuals with a history of trauma, individuals who struggle with autism spectrum disorder, fetal alcohol spectrum disorder
  • Ability to transfer knowledge and awareness into behaviors and expectations recognized as appropriate by members of identified cultures and by people with lived experience
  • Experience in case management and carrying a case load
  • The ability to be energetic, creative with excellent time management and organizational skills
  • Exceptional communication skills, both verbal and written
  • Proficiency using computers including Microsoft Office, Outlook, and Excel
  • Valid Class 5 Driver’s License with access to a vehicle
  • Knowledge of American Sign Language (ASL) is considered an asset

 

 

The salary range for this full-time 70-hour biweekly position is $26.28 to $33.90 per hour, dependent upon education and/or experience.  A full benefits package (which includes Health and Dental Benefits, Long Term Disability Benefits, and Pension Plan) is provided for this 1-Year Term position.

The successful candidate will be subject to Criminal Record, Child Abuse Registry, Adult Abuse Registry, and Prior Contact Checks. A satisfactory Driver’s Safety Rating will also be required.
