New Directions is a non-profit social service agency offering a wide range of resources and services that provide supports in a variety of settings and foster people’s hopes and dreams in their communities.
Adult Family Supports & Outreach Services (AFS) is a support option offered by New Directions and provides services for adults living in various settings including family homes, transitional or long-term care facilities.
Responsibilities of the Case Manager include:
- Working with individuals and their networks of support to coordinate services, including planning, finding resources, making referrals and advocating for services
- Building relationships with the person receiving support, their families and networks of support, and establishing or maintaining connections with culture of origin.
- Directing support staff to develop and meet the needs and goals of the individual receiving services
- Working with collateral services
Applicants must have:
- B.S.W./B.A. or related social services education such as Family Therapy, Human Ecology and Indigenous Studies
- Minimum of 5 years’ experience working with adults with a label of intellectual/developmental disability and with adults in a community setting
- Demonstrated knowledge of person led practices, knowledge of Deaf and Indigenous Cultures
- Trauma Informed Support
- Ability to use the Microsoft Office suite of products, including Word, Excel, Outlook and SharePoint
- Excellent communication skills including mediating difficult conversations and conflict resolution
- Critical thinking and problem-solving skills
- Excellent time management and organizational skills
Other Qualifications considered an asset include:
- Fluency in American Sign Language
- First Aid Certification
- Nonviolent Crisis Intervention training
- Experience supervising staff
The salary range for this 70-hour biweekly position is $26.28 to $33.90 per hour, dependent upon education and/or experience. This is a term position, ending September 27th 2024.
The successful candidate will be subject to Criminal Record, Child Abuse Registry, Prior Contact, and Adult Abuse Registry Checks. A recent Driver’s Abstract and Driver’s Safety Rating must be provided. Access to a vehicle is required.
