About the Opportunity
Currently, Homes First is looking for multiple dedicated Intensive Case Management Workers to join our team in Toronto, Ontario on a full-time basis.
As our new Intensive Case Management Worker, you will support tenants in their housing by providing wrap around case management supports to assist clients to maintain their housing, as well as optimal health and mental health stability, while adhering to the mission statement and policies and procedures of Homes First Society.
More specifically, your responsibilities will include:
- Ensuring that data collected during intake is shared with co-workers in your group to assist in determining the clients’ needs and developing a plan of action with the clients
- Completing and utilizing assessment tools for the clients
- Developing, implementing and monitoring an individual case management plan and crisis plan for each client with other Intensive Case Management Workers in your team
- Responding to conflicts, utilizing crisis intervention and prevention when required
- Developing relationships with clients to provide support and housing stabilization
- Accompanying clients to appointments based on their individual case management plans
- Working with other Intensive Case Management Workers to develop and provide referrals for specialized support to clients based on client’s goals, needs and choices
- Regularly visiting clients and updating the assessment of support and stabilization needed
- Facilitating connections with primary health care physicians and nurse practitioners to help clients benefit from medical care
- Exploring employment opportunities with clients in your team
- Working with clients in regards to maintaining living spaces in order to prevent unstable relationships with their neighbours and landlords (i.e., hoarding issues or pest control issues)
- Working with the team to ensure that income documentation is collected from clients on time
- Calculating the rent according to RGI guidelines
- Designing, scheduling and participating at onsite and community events to ensure client’s involvement
- Being familiar with Canada Revenue Agency processes in order to assist clients in applying for Canada Pension Plan, Old Age Security and/or Guaranteed Income Supplement
- Counselling clients and connecting individuals to psychiatrists for proper assessments, evaluation and therapy
More About You
In order to qualify for this role, you will have a minimum of three years experience with a related post-secondary field of education, or an equivalent combination of education and experience. Certifications in CPR, First Aid and Crisis Prevention and Intervention are all assets.
As our ideal candidate, you'll have demonstrated ability to utilize case management software systems and Microsoft Office, along with the ability to compile data and prepare statistical reports. You'll have an understanding of anti-discrimination practices, diversity issues, and systemic reasons for homelessness. Your experience with conflict resolution and negotiation, combined with your knowledge of mental health and addiction issues will be assets for this role. Communication skills, and the ability to draw appropriate professional boundaries are required, along with experience working with individuals who lack primary healthcare and/or struggle to maintain medication regiments.
You will have demonstrated knowledge of food access programs, along with services in the community such as life skills programs, CCAC and programs that run “extreme clean” services, and experience with harm reduction approaches and the necessary referrals to allow individuals the option to adopt the approaches into their daily lives. You have the resources to connect clients to Ontario Works, Office of the Ontario Public Guardian and Trusteeship, Ontario Disability Support Program, and the knowledge for accessing The Housing Stabilization Fund, expertise in the Residential Tenancy Act legislation, and experience in navigating the criminal justice system on all levels.
Please note that this role requires the ability to lift 20 pounds, as well as the ability to work one evening per week.
