About the Opportunity
Vancouver Coastal Health is currently looking to add a Patient Care Coordinator to our team in Bella Bella, BC.
In this role, you will practice while following the standards of professional practice and code of ethics as outlined by the College of Registered Nurses of British Columbia (CRNBC). As you work you will operate within a patient, client, resident, and family-centred care model while adhering to the vision and values of the organization.
As our new Patient Care Coordinator, you'll coordinate patient care activities and provide leadership and guidance to designated staff.
Some of your key responsibilities within this role will include:
- Facilitating efficient patient flow and patient access within and between units, programs and clinics.
- Assisting the Manager by performing administrative duties and coordinating staffing, scheduling and clinical research activities.
- Operating as a clinical resource for staff by advising training and developing an education plan.
- Setting measurable personal performance goals in collaboration with the Manager for professional development.
- Scheduling educational sessions by collaborating with interdisciplinary team members.
- Developing, reviewing and updating learning resources.
- Promoting Knowledge Translation activities and evaluating results.
- Conducting competency assessments and attending panel interviews for the selection of new team members.
- Supporting with therapeutic interventions and problem solving, as well as providing clinical guidance and information on complex cases.
- Evaluating patient care products/equipment and cataloging new equipment purchases.
- Taking responsibilities in accordance with health and safety requirements.
More About You
In order to qualify for this role, you will need to have completed a nursing certification or program combined with three years of recent related experience. You’ll also join us with one year of supervising and administration experience as well as one year of experience in teaching or group facilitation. Please note that an equivalent combination of education and experience may be considered.
As our ideal candidate for this role, you will also have a current practising registration with the College of Registered Nurses of British Columbia (CRNBC) and experience with CPR techniques. Your clinical knowledge and your ability to provide leadership, supervision, work direction and consultation will ensure your success in this role as you also work to establish a supportive and effective learning environment.
The ability to multi-task, prioritize projects in emergency situations and guide others is essential to deliver quality patient care but also to support staff through transitions.
Above all, you’ll have strong verbal and written communication skills to deal effectively with clients and their families, coworkers, physicians, other health care staff, and external agencies in a courteous, clear, and concise manner.
