Make a Difference Within Our Community

Currently, the Mississaugas of Scugog Island First Nation (MSIFN) is looking for an experienced Community Health Representative to join our community and work with our members and social services team in Scugog, ON on a fixed-term contract (6 months) basis. The hours of this role are Monday to Thursday, 8:30 am to 4:30 pm, and Friday 8.30 am to 1:00 pm. Occasional evening and/or weekend work may be required to meet operational needs.  

JOB SUMMARY:

Under the direction of the Health & Social Services Manager, the Community Health Representative (CHR) supports the entire health team and serves as a key liaison between the team and the community. The CHR is responsible for delivering health promotion, prevention, and education initiatives for the Mississaugas of Scugog Island First Nation. This role encourages community engagement in health programs, fosters improved health outcomes, and empowers individuals to take an active role in achieving and maintaining optimal health. 

ESSENTIAL FUNCTIONS:        

• Design and deliver community health programming through educational workshops, and interactive activities that promote wellness and disease prevention
• Foster active community engagement by encouraging participation in health education, awareness campaigns, and promotional events
• Coordinate specialized health workshops in collaboration with internal staff and external health professionals to address specific health topics or needs
• Collaborate with interdisciplinary health teams—both within the organization and with external partners—to ensure culturally appropriate and effective delivery of health information
• Provide support for individuals and families in understanding and adopting health-related practices aligned with current initiatives
• Serve as a trusted source of health information for the community, providing guidance on where to access reliable resources related to health promotion, disease prevention, and available health services
• Promote a safe and healthy environment by implementing awareness programs that address public health concerns and encourage healthy living
• Advocate for equitable access to health programs and services at both the individual and community levels by working collaboratively with internal health staff to ensure responsiveness to the unique needs of the community
• Develop and implement new health programming, including assisting with proposal writing and funding applications when necessary
• Educate the community on safe health practices, proper use of health services, and navigating the healthcare system effectively
• Disseminate information about available health supports, including local providers, services, and referral pathways
• Provide guidance on health benefits, including Non-Insured Health Benefits (NIHB), CINUP, and other relevant programs or organizations.
• Make available preventative health sessions, organizing and facilitating workshops focused on early detection, lifestyle changes, and wellness management
• Manage program budgets, including preparation, reconciliation, and reporting in accordance with organizational and funding requirements
• Participate in the planning, development, and evaluation of health programing, offering input and consultation on proposed initiatives to ensure alignment with community needs and best practices

PAY RATE:                               $29.18 - $33.98 (Pay Grade I) 

 MINIMUM QUALIFICATIONS: 

• 1 year certificate in a health-related field
• 1 year work related experience, 3 years total work experience or equivalent training and experience as determined by the hiring committee

SPECIALIZATION REQUIRED: 

• Valid driver’s license and $1M liability insurance
• CPIC acceptable to position upon conditional offer
• Current CPR and First Aid Certification
• Knowledge and understanding of Indigenous culture, traditions, teachings, community dynamics
• Strong written and verbal communication skills
• Excellent interpersonal skills when working with colleagues, community and external partners
• Detailed oriented and excellent organizational skills
• Ability to analyze problems, recommend and implement solutions
• Knowledge of concepts, principles and practices of community health
• Strong working knowledge of Microsoft Office software suite

CONSIDERED AN ASSET:

• College Diploma Post-Secondary diploma or degree in a health-related field
• Experience in community health   
• Work experience with a First Nation or Indigenous Organization
